This form has two sections; organizations detail section where you capture the details of your organization and the data capture section

When filling in the data section; Capture employees (workers) category then for each category key in details before and after the COVID-19. Once done with one category, click (+ Add Another) icon on the bottom left of the form to create another form for another category. Repeat this process untill all categories are captured then click on submit
